1. copy of business license -required plus one of the following:
2. a voided business check with acct no blacked out
3. a recent purchase invoice from another vendor as long as your business info appears on it as well
4. a yellow pages ad
5. a badge from a trade show attended during current year
 
Initial min order is $300.00 and orders thereafter are $100.00
